| OUR INSURANCE POLICIES |
We accept most insurance plans, however, the amounts paid and the number of visits allowed are variable and
depends on the policy chosen by your employer group. As a courtesy, we check your policy benefits and if an
authorization is required but we do recommend that you check your policy and contact your carrier if you have
questions.
Please note that if your insurance company requires a co-payment for services, the payment is required at the
time of each visit. If your insurance company requires a deductible and/or co-insurance a flat rate will be
collected at each visit to cover these charges. If there is a credit on your account, you will receive a check after
your therapy has been complete.
For all patients, a prescription for physical therapy is required from a physician. This is a requirement of the
state of Virginia. It is not associated with your health insurance.

**Medicare - In order to continue physical therapy, Medicare patients must schedule
return visits with their physician every thirty days and obtain a new prescription for
physical therapy.
***Medicaid HMO - We can accept Medicare HMO as primary. We are contracted to
accept Carenet through Southern Health and Healthkeepers Plus through Anthem.
****Workman's Compensation - In order for us to file claims efficiently contact
information (including filing address) and a claim number are required at the time of the
first visit.
